All appointments at this time are video.  Given that Dr. Keyashian is licensed to practice medicine in California, patients must be located within California for all appointments.  If at the time of your appointment you are at a different address than the one on file (e.g. work, family, etc.), you will need to provide that address at the time of your appointment in case of emergencies. You must also be at an appropriate location for the appointment (e.g. quiet, private, good internet connection, safe, not in a moving vehicle, etc.).  If an appointment needs to be cancelled because you are out of California or not at an appropriate location, you will be responsible for the cost of the appointment.  Thank you for your understanding.

In order to request an appointment with Dr. Keyashian please click the "Book Online" button below.  Clicking on this button will take you to ZocDoc.com, a secure and confidential website where you can request a new patient appointment. Please complete the electronic check-in form on this website where you submit personal information over a secure and confidential connection to Dr. Keyashian.

If you would like to use your health insurance to see Dr. Keyashian, please email a copy of the front and back of your insurance card to drnissak@drnissak.com or upload it to your appointment request on ZocDoc.com as soon as possible. Dr. Keyashian is a contracted provider with Anthem Blue Cross. If you have other health insurance you can send or upload your insurance card, we can check your out of network benefits, and we will bill your insurance as a courtesy but you will be responsible for the cost of the appointment at the time of the appointment; your insurance will reimburse you based on your benefits.

Please also note Dr. Keyashian has opted out of Medicare and thus claims cannot be submitted by the office or the patient to Medicare.  If you have Medicare please notify Dr. Keyashian so a private contract can be created.   In order to confirm the appointment we will need the front and back of your insurance card and the completed and electronically submitted ZocDoc.com check-in form described above. Dr. Keyashian may also need to speak with you briefly to ensure that she is the best psychiatrist to treat you.

Once we confirm your appointment we will email you the location of the virtual appointment through Secure Video. Once you receive the email please go to Secure Video to read and electronically sign The Office Policies and Procedures and The Acknowledgement of Receipt of the Notice of Privacy Practices Forms (also found below). We also ask all new patients to sign an arbitration agreement but this is not required for evaluation (please see below for further information).  If one of your healthcare providers has referred you to Dr. Keyashian or if it will be important for Dr. Keyashian to communicate with one of your providers (e.g. your therapist, primary care provider, etc.), please also fill out The Authorization for Release of Health Information Form. The Notice of Privacy Practices is for you to keep.

I ask all of my patients to sign an arbitration agreement but it is not required for evaluation.  In my practice of medicine I, of course, aim to provide the highest quality of care to all of my patients and to prevent disputes from arising.  However, I believe that if disputes do arise, arbitration is the best method to address them for all involved parties.  Please see below for information on arbitration and a copy of the arbitration agreement.  Please let me know if you have any questions.
